Understanding the connection between severe stress and dementia symptoms

Severe stress can sometimes appear alongside a decline in cognitive functions that may look like early onset dementia. This is both confusing and anxiety-provoking for the person experiencing the decline and those around them. Sudden memory lapses, difficulty staying focused and concentrating on tasks, alongside impaired or slower thought processes, are common in both conditions; however, the underlying mechanisms differ significantly. If you are concerned that you are developing dementia, understanding why severe stress looks similar can help identify the appropriate approach to treatment and support.

How severe stress affects the brain
The stress response system initiates and coordinates a cascade of biological instructions sent from the brain to our organs, the endocrine (hormonal) system and our immune system. With the primary role being survival, this system releases stress hormones, such as cortisol, adrenaline and noradrenaline, ensuring that energy and resources are available for the sympathetic “fight or flight” nervous system.
This complex and yet, fine-tuned response involves feedback loops to ensure homeostasis (balance) is restored once the perceived threat has been dealt with. This ensures that energy is available for normal “housekeeping" processes such as digestion and restoration of energy. When stress becomes severe or chronic balance is not readily restored, the ongoing release of stress hormones can then disrupt brain function, especially in areas responsible for memory and clear, rational thinking:
Hippocampus: The hippocampus plays a key role in forming new memories and the retrieval of memories. High cortisol is toxic to this region and causes atrophy (shrinkage) leading to memory problems. Fragmented memory can be seen when recalling traumatic events.
Prefrontal cortex: This brain region controls attention, decision-making, emotional regulation and helps us solve problems (this is how we got rockets to land on the moon.) Stress impairs its function, leading to challenges with executive functions, including focus, attention and cognitive flexibility.
Amygdala: The amygdala is a gatekeeper for threat-detection. It is involved in emotions like fear and anxiety, but chronic or severe stress can cause it to become overactive, interfering with clear and rational thought processes.
These changes explain why people under severe stress may struggle with tasks that require memory, focused attention and logical reasoning; resembling some dementia presentations.
Common symptoms shared by severe stress and dementia
Both severe stress and dementia can cause similar cognitive, emotional and behavioural symptoms, making it hard to distinguish between the two without professional evaluation. This includes, but is not limited to:
Memory loss: Forgetting recent events, people's names and words mid-sentence.
Confusion: Difficulty processing information or following the flow of conversations.
Reduced concentration: Trouble focusing on tasks or conversations or difficulty initiating tasks.
Slowed thinking: Taking longer to process information or respond to questions.
Mood changes: Increased irritability, anxiety or depression.
Sleep disturbances: Insomnia or restless sleep affecting cognitive function; this can look like brain fog.
While these symptoms overlap, the causes and progression are very different with distinctive mechanisms underpinning them. Stress-related symptoms will often fluctuate and also improve with stress reduction strategies; conversely, dementia is characterised by the progressive loss of brain cells seeing symptoms worsen over time.
Why stress symptoms can be mistaken for dementia
There are a number of reasons the overlap between severe stress and dementia can cause confusion:
Sudden onset: Stress-related cognitive problems can appear quickly, especially after a traumatic event or prolonged exposure, similar to how some dementia symptoms may emerge.
Severity of symptoms: Intense stress can cause significant memory and cognitive difficulties mimicking early declines seen in dementia.
Lack of awareness: People under stress may not recognise their cognitive decline, this is similar to dementia patients who may be unaware of their reduced cognitive abilities.
Overlap with depression and anxiety: These conditions often accompany stress and can worsen cognitive symptoms, further resembling dementia.
Because of these overlaps, it is essential to talk to your GP if you have any concerns, if required they will refer you to a specialist for more indepth assessments, to rule out dementia.
How to differentiate between severe stress and dementia
Distinguishing stress-related cognitive issues from dementia requires careful evaluation of symptoms. Some can include:
Symptom timeline: Stress symptoms often develop rapidly after a stressful event, while dementia usually progresses more slowly.
Reversibility: Cognitive changes resulting from stress may improve with rest, stress management and lifestyle changes. Dementia symptoms typically do not reverse; the dementias are neurodegenerative meaning there is a progressive decline in the health of the brain with region-associated functional changes.
Memory type affected: Stress typically impacts short-term or working memory and attention, while dementia affects multiple memory types, including procedural memory.
Additional signs: Dementia may include physical symptoms like changes in posture, gait or coordination, which are not caused by stress.
Neuropsychological testing: Detailed cognitive tests can help identify patterns typical of dementia rather than pseudo-dementia attributed to stress-related changes.
Brain imaging: MRI or CT scans can reveal brain changes consistent with dementia; however, they would usually appear normal in stress-related cases.
Practical steps to manage severe stress and protect cognitive health
If severe stress is causing dementia-like symptoms, addressing stress is essential to improving cognitive function.
Identify stressors: Recognise and reduce sources of stress where possible.
Practice relaxation techniques: Meditation, deep breathing and yoga can lower stress hormone levels. It helps to know which technique will work under different circumstances, I teach classes for this.
Maintain a healthy lifestyle: Regular exercise, balanced diet and sufficient sleep support brain health.
Seek social support: Talking with friends, family or support groups helps reduce feelings of isolation. Loneliness affects mood and cognitive functioning; this is not discussed enough in my opinion!
Professional help: Coaching, therapy or counselling can provide coping strategies and treat underlying anxiety or depression. Cognitive exercises: Puzzles, reading and memory games stimulate the brain and improve function. These techniques are mindful exercises and mindfulness is important for stress management.
These steps not only help reduce stress but can also protect against long-term cognitive decline.
When to see a doctor
It is noteworthy that chronic unmanaged stress and neuroinflammation can increase the risk of developing dementia; therefore, symptoms should not be ignored. If memory or thought processes persist despite stress management or if symptoms worsen, it is important to seek professional advice from your GP. Early diagnosis of dementia or other medical conditions can lead to better care and planning.
Signs to watch for include (but not limited to):
Increasing forgetfulness that affects daily life
Difficulty performing familiar tasks
Problems with language or communication
Changes in personality or behavior
Loss of coordination or balance
Getting lost in familiar places
A thorough medical evaluation can clarify the cause and guide treatment.
